I have 2 questions.

On 8/20/20, I walked out of the Centrepoint Pharmacy with a prescription for FUROSEMIDE, 20 MG TABLET,
quantity 3. (Copy of the label is attached)

Question 1)
I would like to know the name and the professional status (license number) of the pharmacist who dispensed that prescription.

On 8/25/20, I walked out of the Centrepoint Pharmacy with a container of FUROSEMIDE, 20 MG TABLET, quantity 1. (Copy of the label is attached)

Question 2)
I would like to know the name and the professional status (license number) of the pharmacist who dispensed that tablet/prescription (I’m not sure if the word “prescription” applies in this case).
