Kaiser Permanente Mail Order Pharmacy, OSP 0007076 For your convenience, this letter (including all of the
attachments) is
published on the Internet at: re: Tamsulosin 0.4 mg 24hr er cap It is my belief that I have a right to expect my prescriptions to be filled by a licensed pharmacist, from a licensed pharmacy.
And it is my belief
that I have the unequivocal right to know the name of the pharmacy, and the pharmacist, so
that I can look them both up (verify) on the state board website: If there is a record of DISCIPLINARY history listed for either the pharmacy or the pharmacist, I want to know. I have a right to know. I have called the Kaiser Permanente Mail Order Pharmacy and asked to speak to a pharmacist many times. I always ask the question, are you a pharmacist? When they say yes, I ask them to tell me their name. They always refuse. Every time. I live in Colorado. It is just a voice on the phone. If I don't know their name, I cannot verify that they are a pharmacist. They insist that they really are a pharmacist, I should just trust the voice on the phone. How do you feel about that?
I have published a copy of RX 3078 0111 3654 (8/1/20) at: 1) What is the name of the pharmacist? 2) What is the name of the pharmacy? 3) What is the name of the drug? 4) I called on 3/16/21 and asked to speak to a pharmacist. The person I spoke to insisted that they were a pharmacist, but they refused to tell me their name. Can you confirm that I spoke with a pharmacist on that date? If so, what is the name of the pharmacist?
