I live in Aurora, Colorado. I called 360 236 4700 today with questions about the attached prescription , and they referred me to this email address.

I am accustomed to seeing the initials of the pharmacist on the label. On this label, Optum has printed the name of the Pharmacist, E Furtado. Is there a law or a rule in Washington that requires the full name, not just the initials? If so, I would like to read the law/rule. Can you provide me with a citation, and/or a copy of the law? And/or the contact info of someone I could speak to. I would like to pursue the possibility of implementing the same rule in Colorado. Who can I talk to?

re:  Update Please?

For your convenience, this letter (including all of the exhibits) is published on the Internet at: mkgappraisal.com/letter2023_06.htm

I'm in Colorado. I would like to write a letter, and then send a copy to each member of the California State Board of Pharmacy via the US Mail. I found the web page :

https://www.pharmacy.ca.gov/about/board_members.shtml

and I read the following:

The California State Board of Pharmacy comprises 13 members appointed to four-year terms. Members can serve only two terms. The Governor appoints seven registered pharmacists (“licensees”) who reside in different parts of the state and four public members. The Senate Rules Committee and the Speaker of the Assembly each appoint one public member. Public members are not licensed by the Board of Pharmacy.

  1. Seung Oh, PharmD, President, 2023
  2. Jessica Crowley, PharmD, VP, 2025
  3. Trevor Chandler, Public Member, Treasurer 2025
  4. Renee Armendariz Barker, PharD, 2023
  5. Indira J. Cameron-Banks, Public Member, 2024
  6. Jose De La Paz, Public Member, 2024
  7. Kartikeya Jha, R. Ph., By Who?, 6/1/24
  8. Jignesh Patel, R.Ph., 2026
  9. Maria D. Serpa, PharmD, VP, 2026
  10. Nicole Thibeau, PharmD, 2024
  11. Jason Weisz, Public Member, 2024

  1. Kartikeya Jha was appointed by who? Term expiration listed as 6/1/24, which is very good. All of the other expiration dates should be in this same format. You can do this.

  2. On June 5th, 2023, you updated the page and removed Ricardo Sanchez who was listed with a term expiration date of 2022. For all of the members, it would be better (more user friendly) to list the expiration date in the format 6/1/22. Please be a little more specific. I want to know on what day in 2022 did Ricardo Sanchez term expire. Or was his last day in 2023? And why did it take until June 5, 2023 for it to be reflected on the Member Web Page? Was it because Anne Sodergren was asleep at the switch? If not Anne Sodergren, then who is responsible for the content of this Web page?

  7. Kartikeya Jha, R. Ph., By Who?, 6/1/24

I am received an email dated April 18th, 2023, from Medicare.gov.  The email says:

Thanks to the prescription drug law  that went into effect this year, even more vaccines are free for people with Medicare Part D.  Examples of vaccines now covered under Medicare Part D include:
  1. Shingles
  2. Tetanus/diphtheria (Td)
  3. Tetanus/diphtheria, and pertussis (whooping cough) (Tdap)
  4. Hepatitus A
  5. Hepatitus B
Sincerely,
The Medicare Team

I received the Hep B vaccine in the form of two injections at my local King Soopers pharmacy.  The first shot on March 12, 2023, and the 2nd shot a month later on April 13th.  In order for me to receive the shots, it was necessary for me to first sign my name to a Vaccine Consent Form , which contained (in small print) the statement:

I authorize Kroger to submit a claim for reimbursement on my behalf to Medicare.  If the claim is denied, I understand that I will be responsible for payment.

Aetna refused to pay.

After receiving your Aetna Formulary Letter dated 3/14/23, I read the instructions.  I then called (800) 414-2386 and spoke with your people.  I tried this six different times.

Aetna is not acting in good faith.

The Joe Biden Executive Order  dated 10/14/22, the Inflation Reduction Act (IRA), and the Medicare.gov email dated 4/18/23, all make a compelling argument that Aetna is required by law to pay my Hep B vaccine fees.  Because of Aetna's stubborn refusal to obey the law, it has been necessary for me to carry a $330 liability on my personal balance sheet.

Karen Lynch, quit stalling.  Pay up, and then send me proof that you did.

If you are unable, or unwilling to send me copies of cancelled checks, or some other proof of payment, then issue a check in the amount of $330 payable to me.  Stop wasting my time.
