Amy Hinds6/26/xx
South Logan Family Practice
For your convenience, this letter is published on the Internet at: I had an appointment on Friday, January 15th, 2021 with Amy Hinds, my new Primary Care Provider (PCP). I left with a hard copy prescription for a quantity of 30 phentermine pills.
Amy told me I could buy the pills right there at the clinic. I
was surprised. I asked how much it would cost. Amy did not know, she had to go
ask someone.
I had asked for a quantity of 90 pills, but Amy would only write the scrip for 30. At a subsequent appointment, Amy showed me that when she tried to enter the number 90, her computer changed it to 30. Her computer insisted on a quantity of 30. I did not want to make an issue out of the quantity. Amy told me it was Office Policy that I would be required to make an appointment and visit the office every time I wanted to refill this scrip. There would be no refills over the phone. So I made an appointment for Wednesday, February 3rd, 2021. I then left the doctor's office with a hard copy prescription for 30 phentermine pills. Later that day, I gave the scrip to the pharmacy at King Soopers, and asked them to get it ready while I did some shopping. Five minutes later, while I was shopping, I heard my name on the loud speaker. I went back to the pharmacy, and they told me they could not fill the scrip because it had no date. It was 4:45 pm on Friday, the start of a 3 day weekend. I should have carefully proof read the prescription before leaving the doctor's office. The prescription is not dated. My street number and street name are both typed wrong. The name Amy Hinds is not typed on the scrip anywhere. Presumably, the pharmacy finds the prescriber name using the DEA number. I'm guessing. Monday, January 18th, 2021, was MLK day. Tuesday, January 19th, 2021, was the first day the doctor's office was open. I took the undated scrip and drove to the Family Practice office, a 30 minute drive each way. I had to wait to see Amy. I could not just walk right in. I handed Amy the piece of paper and told her the pharmacy would not fill the scrip because it was undated. She picked up a ball point pen, wrote in the date, and handed the piece of paper back to me, without saying a word. I then drove to the pharmacy and filled the prescription. King Soopers charged me $12.69 for 30 pills.
