Phone Abuse Outline
2023

1) Data Breach -- Nations Benefits notified Aetna on 2/9/23. Nations Benefits notified me with letter dated 4/27/23.

2) Sharon Phillips called me on 6/8/23. This caused me to call customer service (the number on my insurance card) and change my phone number / make do not call request. And then, confirm with letter.

3) Gabriel Doe called me on 06/27/23 to let me know he had received my do not call letter, and that he was on the case.

4) A machine/robot named "Susan", of Signify Health, left a voice mail on 7/6/23. Call.

5) A machine from CVS Caremark Pharmacy called on Friday 7/28/23 at 12:15 pm. I was driving the car. The machine said it had important information for Philip Rice. Phone records say the call was from (855) 418-9359, lasted 1 minute. The machine asked me for the last 4 digits of the ID number on my insurance card. I hung up.

6) Leigh Doe, CVS, called me on Wednesday 8/2/23. The call lasted 36:31, from (877) 864-7747.

7) I spoke to SHIP on Friday 8/18/23. The call lasted 1:45:04. The call had two parts, A very long first part, and a short second part. The second part covered the phone abuse. I can summarize the conclusion of SHIP by saying that this was my second try, and they are at least useless. and perhaps worse than that. I think maybe I should have asked this guy (David) who is the president of the United States? I think he might have had difficulty saying anything other than Trump. Just a guess, I could be wrong. That's the way it came across to me.

8) Data Breach / HCPF 5/28/23

Letter Dated 8/11/23 from Colorado Department of Health Care, Policy & Financing ( HCPF ). Received 8/18/23. Envelope .

HCPF oversees Colorado's Medicaid program, Child Health Plan+, and other un-named Colorao health care programs.

On or about 5/28/23, an unauthorized actor accessed my personal information. Specifically my full name, Social Security number, Medicaid ID number, Medicare ID number, date of birth, home address and other contact information, demographic or income information, clinical and medical information (such as diagnosis/condition, lab results, medication, or other treatment information) and health insurance information.

2) Call from Sharon Phillips. CVS employee in Arizona. She wanted a phone number for the King Soopers pharmacy on Parker Rd in Aurora. When I tried to tell her that I had gone to a great deal of trouble to make that information available on the Internet, and offered to tell her how to easily find it, she wanted nothing to do with that. She insisted that I needed to stop what I was doing and go get the phone number so that she would not need to be bothered hunting for it.

I asked for her name, mailing address, and phone number. She told me.
I mailed a letter to the address she gave me. A month later it was returned by the Post Office because of a bogus address. I called Sharon on the phone. The call lasted 24:25. She gave me a corrected address, but she told me the letter would not come to her. On 7/12/23, I mailed the letter to the corrected address:

we have been having issues with Mail Order Pharmacy and a few here in Colorado KP region that are not filling 90 as requested. I am working on this problem and since Mail Order is from California, we do not have any control over their dispensing. However, this was to be addressed sometime this month.
